Criteria for Field Trip
A student may NOT be suspended at all from school or bus.
A student may NOT be assigned ISS for more than one consecutive day (i.e. 2 days in a row).
A student may NOT have an average below 60% for the second semester at the time of the trip.
A student may NOT have more than 4 unexcused absences during the second semester.
All fees must be paid or arrangements made with school administration prior to the field trip.
This form must be completed and returned to be eligible for the trip.
