Records/Transfers

Steps to Enroll a Student at UCMS:
1.  Complete Online Enrollment - you will need SS number/card, birth certificate, addresses, emergency contact info., and other important information.
2.  Provide all REQUIRED documents:
--IMMUNIZATION RECORD - Current and on Kentucky Form.
--PHYSICAL - Current as of 6th grade on Kentucky Form.
--Certified Birth Certificate (issued by the state - not hospital)
--Social Security Number
--Eye/Dental - not required, but we will take copies of those for Health Record purposes.
 
 - Note - If you are enrolling from out-of-state, immunization records must be transferred onto a KY Form; physicals must be current (6th grade mandatory physical - current 1 year prior to 6th grade enrollment) and on Kentucky forms as well.
 
To make the enrollment process easier and faster, please have copies of transferring academic documents such as current grade card, latest assessment results/test scores, copies of any IEP/504/GSSP (gifted student services plan).  If you do not have copies, we will obtain these documents from the transferring school.
